Support for children and young people moving between phases of education.

Transition from an early years setting to school and from primary school to secondary school and then to further education is a vulnerable time for all children and young people but especially for those with special educational needs and disabilities.

When we invited early years settings to make an entry to the Local Offer, one of the questions they were asked to respond to is:

How will the setting support my child in transferring to the next stage of education? You should find the answers to this question in each setting’s service information on the Local Offer.

When mainstream and special schools submitted their entry to the Local Offer, one of the questions they were asked to respond to is:

How will the school support my child to join the school and how will the school support my child in transferring to the next stage of education? You should find the answers to this question in each setting’s service information on the Local Offer.

Early years settings and schools provide a lot of support for children and young people transferring between all stages of education.

Additional support from Outreach Providers is commissioned by the Early Years Early Intervention Panel, The Primary Consortia and the SEN and Early Intervention Panel 

Provision

The Special Educational Needs Inclusion Support Service (SENISS) provide a service for children in the early years delivered by Inclusion Support Workers who work with Specialist Advisory Teachers. 

The outreach team at Springwood Heath Primary School support the transition of children with physical difficulties and or fine and gross motor difficulties between all phases of education. 

The Sensory Service supports children and young people of all ages with a sensory impairment to move between phases. 

Millstead Primary School and Princes Primary School provide outreach for children moving between the early years/ primary phases with Severe Learning Difficulties and can support transition planning. 

Redbridge High School and Palmerston High School provide outreach for children moving between the primary/secondary phases with Severe Learning Difficulties and can support transition planning. 

Abbot’s Lea School provide outreach for children and young people with Autistic Spectrum Disorder and can support transition planning between all phases. 

 

Bank View High School and Childwall Abbey School provide outreach for children with complex learning difficulties moving between the primary/secondary phases and can support transition planning.

Clifford Holroyde Specialist College, Hope School and Ernest Cookson School provide outreach for children with social, emotional and behavioural difficulties and can support transition plans. 

Barlows Primary School provides outreach for children in the early years with a range of needs and supports children moving between early years/primary phases. 

The Social Inclusion team provide support for children with fixed term exclusions and at risk of permanent exclusion moving between the primary/secondary phases with a six week transition programme delivered by the Student Support Centres in the summer term. The STAR (Support Transition and Reintegration) workers provide support during the first term of a new school or for however long it takes to ensure vulnerable pupils settle.

SENCo Transition Day

This is held during the summer term to support the exchange of information about children with special educational needs and disabilities moving between Key Stages 2 and 3. School Improvement Liverpool invites SENCos from primary and secondary schools to the event to share information that will support a smooth transition to the secondary school. This has been disrupted due to the pandemic, but we will update when we are aware of new developments
